What you need and how to answer questions

Even if you don’t choose us, our priority is always making sure that you choose the right provider who can support you every step of the way. Sapphire Living is all about your homecare, so if you choose to speak to a number of providers before making your decision, the below checklist is there to help.

→ Step 1 Eligibility

Eligibility is based on needs and meeting My Aged Care criteria. You can register online with My Aged Care www.myagedcare.gov.au/assessment/apply-online To find out if you meet the criteria, My Aged Care will ask a few questions about your current situation to see if you need help with some everyday tasks, or if you prefer, we can do this for you over the phone.

Here are some of the questions:

  • Getting out of bed or out of a chair
  • Walking
  • Toileting, bathing and/or showering
  • Getting dressed
  • Preparing a meal
  • Eating a meal
  • Taking or remembering to take medicine/s
  • Basic housework
  • Driving or taking public transport
  • Shopping for groceries
  • Managing money and paying bills
  • A stay in hospital
  • A slip, trip or fall
  • An illness or disease
  • A sudden weight change
  • Anxious or depressed
  • It’s difficult to remember things
  • Any recent changes in behaviour
  • Lonely or socially isolated
  • Friends or family no longer able to support you
  • Your living arrangements have changed or about to change
  • Any financial hardship
  • Concerns about safety when alone

→ Step 2 Providing your details

You will be asked to provide some further information about yourself.  

These will check your identity and create your record with My Aged Care

→ Step 3 Arranging the assessment

  • Have ready your health details
  • Income tested care fee form
  • Pension number
  • GP contact details
