Home Care Packages are funded by the Government to assist older Australians keep well, safe and independent in their own homes. The funding is paid to a Service Provider (like Sapphire Living) to arrange a package of care services to meet your needs. The Commonwealth Home Support Programme (CHSP) helps senior Australians access entry-level support services at a subsidised cost, to live independently and safely at home. Services can be for community respite or homes services like cleaning or health services like physiotherapy. Often CHSP services are allocated prior to being assigned a Home Care Package
No, CHSP finishes when you accept your Home Care Package. You will be able to have your new services covered by your Home Care Package without having to pay the subsidy.
To get approved for a HCP you need to first register with the Government’s My Aged Care . If you prefer, you can phone us on 1800 773 722 and we can complete your referral and get you started. There are 4 levels of (HCP) which are assigned by the Aged Care Assessment Team (ACAT) according to your needs
Level 1: Basic care needs – approx. $9,000 per year
Level 2: Low care needs – approx. $15,750 per year
Level 3: Intermediate care needs – approx. $34,250 per year
Level 4: High care needs – approx. $52,000 per year
After you have been assessed by the Aged Care Assessment Team [ACAT] as being eligible, the wait time are as below.
Level 1: 3-6 months
Level 2: 12+ months
Level 3: 12+ months

Yes, please contact our friendly team on 1800 773 722 and they will guide you through the process. The process includes you contacting My Aged Care on 1800 200 422 and asking them to reactivate your referral code as you want to switch to Sapphire Living. We will then be able to organise a time for sign-up with you, so we can commence your care services. We will also speak with your current provider to organise the date of transfer of your funds, so there is no gap in your services. We organize everything from start to finish and it’s a really easy process to switch provider.
Once we receive your Home Care Package assignment notification, a dedicated Care Manager will work with you, based on your assessment, to develop a personalised care plan, working within your budget to achieve your goals. A Referral Code is a unique number that you receive when you are assigned a Home Care package. You will find this number when you receive your letter from My Aged Care advising that you are assigned a Home Care Package. This code allows your Provider to access your ACAT assessment with details of your situation and care needs

If the government assesses you as having to contribute to your Home Care Package, then you must pay the income tested fee – it cannot be waived by a Provider. If you have questions on this, please call our friendly Customer Care team on 1800 773 722.
How much you pay for your Home Care Package depends on your circumstances – this can be the services you need and your financial situation. This is a government fee that is used to contribute to your overall services. You may have to pay the following per day but it’s best to give us a call on 1800 773 722 to talk through your current circumstances:
Level 1 – $9.72
Level 2 – $10.28
Level 3 – $10.57
Level 4 – $10.85
Unlike other providers we do not charge an entry or exit fee.
